Academic Year 2010-2011 Annual Faculty Survey

Welcome to the Association of Schools and Colleges of Optometry's Annual Faculty Survey. This web site provides you with the opportunity to add, delete and update data that has changed since the last survey.

Please make all changes no later than Monday, October 25, 2010. The information provided in all parts of this survey should reflect your faculty as of the beginning of the 2010-2011 academic year.

Please designate one person as the survey coordinator in order to ensure that all sections are completed and reported. The coordinator is responsible for providing the log-in information and passwords to the individual(s) responsible for completing the various sections.

The Faculty Survey is divided into five parts: general information, full and part-time faculty survey (includes demographic information), full and part-time salary survey, employee benefits and standard work week, and unpaid faculty head count. The Survey Coordinator must complete part 1, general information, before moving on to the other sections. The Survey Coordinator has the option, within the Survey, to assign separate passwords to access sections two through five. This feature allows the salary information to remain confidential and restricted even from the people who complete the other sections of the Survey. To assign these passwords or update them from the previous Survey, please see "Modify Users and Assignments" on the main menu page (the main selection screen).

Because only faculty IDs are used in the salary section and not names, it will be helpful to print out the data summary from the general faculty survey, which includes both names and ID numbers, when completing the salary section.
